Family Medicine

We are an innovative, health-care team combining the best in patient and family-centered care, education and research. Our primary care team combines the expertise and services of different health-care providers to help you address many kinds of health-care needs. We provide families with a range of comprehensive primary care services at any age.

We are committed to pursuing excellence and innovation in everything we do. We take a collaborative and multidisciplinary team approach to patient care and bring the best of what family medicine has to offer at Mount Sinai to our patients at the Sherman Centre.


The team’s services include both preventive care to help you stay healthy with things like flu shots and wellness workshops, and acute care to help you live well with conditions like diabetes and heart failure.

Patients of all ages are welcome. Some members of the team are providing maternity care to pregnant people, including attending births. Other members have expertise in senior care and care for people who have complex health needs.  The team is also providing community-based mental health care.

Clinic hours are Monday to Friday from 9 am to 5 pm. In addition, the team is pleased to offer patients of the Sherman Health and Wellness Centre access to after-hours care – both through schedule appointments and walk-ins for urgent care – Monday through Thursday evenings and on Saturday mornings.

Prenatal and Parenting Classes

Part of Mount Sinai Hospital, one of the top women’s health-care centres in the world, our Prenatal and Parenting Education Program offers a number of classes and services, all provided by experienced health-care professionals.

Note: You do not need to be a patient at Mount Sinai Hospital to take any of our courses.

Program Schedule Cost

Preparing for Childbirth:

The most popular of our prenatal classes, this course prepares families for the process of labour and birth, caring for their new baby, breastfeeding and more.

One series of two Sundays/month $225.00 per expectant mother and support person

 

Function and Pain

Our Function and Pain Centre (FPC) offers an intensive, personalized program, usually lasting 30-40 treatment days, and integrates both psychological and physical treatments for patients suffering with chronic pain. The FPC works in collaboration with the Wasser Pain Management Centre at Mount Sinai Hospital which provides patient care, education and research in the area of chronic pain disorders.

Funding for these services may be available through the WSIB, motor vehicle accident (MVA) insurers, long term disability (LTD) insurers or employers. Self-pay is also an option.

No formal referral is required.

 

Treatment Description Roles

Functional Restoration Component

  • This component includes a supervised exercise program specifically tailored to the patient’s functional and vocational goals.
  • The Functional Restoration stage provides posturing and positioning, assistive devices and ergonomic equipment when making the transition from the clinical setting to the home or work site.
  • The Kinesiologist helps to improve a patients’ performance with stretches and exercises while monitoring the patient’s motivation and pain behaviours.
  • The Occupational Therapist provides transitional support, in-home functional assessments or treatment, job site analyses, physical demands analyses, ergonomic assessment, return-to-work program design and implementation.

Medical Component

 

  • The medical management includes both initial assessment and reassessment of the patient during treatment as well as attending weekly rounds where each patient’s progress is reviewed.
  • The Medical Consultant, together with the Clinical Pharmacist, reviews all medications and recommends optimal therapy.
  • The Medical Consultant also communicates with family physicians and other specialists when needed, which may entail explaining the program as well as requesting support for their patients’ recovery plan.

Physical Component

 

  • Treatment includes manual techniques that focus on soft tissue and joint dysfunction.
  • Biofeedback is an advanced treatment method using computerized technology that provides instant feedback related to muscle tension, blood flow, and breathing patterns and skin temperature.
  • Massage Therapy relieves the tightness of muscles and releases active trigger points while stretching and anxiety reduction techniques improve mobility.
  • The Physiotherapist/Chiropractor selects the most appropriate therapies for each patient’s needs and recommends a routine that includes education on conditioning, posture, stretches, soft tissue & joint mobilization, and mobility exercises.
  • The Physiotherapist/Chiropractor supervises the Biofeedback Therapist and Kinesiologist.
  • The Registered Massage Therapist and the rest of the team work closely in an integrated program.

Psychological Component

 

 

  • Individualized psychological treatment focuses on the identified emotional and psychological issues to help maximize the patient’s adjustment and functioning.
  • Provides patients insight into the causes of stress, while cognitive behavioural training techniques provide the basis for the pain management strategies taught throughout the program.
  • The Psychologist supervises the psychological aspects of the program and provides recommendations on how best to manage the emotional, cognitive, and personality relevant to the patient’s progress.
  • The Cognitive-Behavioural Therapist assists the patient in recognizing how his or her thoughts and behavioural patterns affect mood and pain levels.

Treatment Symptoms

Acupuncture:

Acupuncture is an ancient Chinese medicine practice that strengthens the immune system by increasing white blood cell count, manages pain by releasing endorphins, and improves sleep by modifying physiological functions of the brain.

  • Disorders of bones, muscles, joints and nerves, arthritis, osteoarthritis, tendonitis, tennis elbow, carpal tunnel syndrome, migraines, dizziness, low back pain, neck and shoulder pain, headaches, muscle and facial spasms.
  • Sleep and stress disorders such as insomnia and fatigue

Massage therapy:

Relieve the tightness of muscles, increasing mobility and over-all wellbeing.

  • Back, neck and shoulder tension, carpal tunnel syndrome, spasms, digestive disorders, fractures, frozen shoulder, headache/migraine, insomnia, post injury rehabilitation, post surgical rehabilitation, pregnancy and labour support, sports injuries, strains and sprains, depression, anxiety, stress, asthma.

Chiropractic Services:

Chiropractic treatment is used relieve a variety of muscle, joint and spinal conditions and focuses on musculoskeletal structure, the nervous system and their effects on the general functioning of the human body.

  • Musculoskeletal pain, back pain neck pain and related stiffness.

Podiatry:

(Including Shockwave and Laser Therapy): Preventing, diagnosing, and treating conditions associated with the foot and ankle.

  • Achilles tendonitis, arthritis, bunion, chondromalacia, diabetes, ilio-tibial band, morton’s neuroma, plantar fasciitis, shin splints

Physiotherapy:

Often used when discs, ligaments, muscles, or other joint tissues are injured.

  • Neck and back pain; pelvic pain; joint injuries including temporomandibular dysfunctions, anterior cruciate ligament tears or rotator cuff issues; carpal tunnel syndrome; headaches; peripheral nerve impingements; scoliosis; sport injuries; plantar fasciitis; osteoarthritis; and pre and post operative rehabilitation.

Psychological Counselling:

Individual therapy focusing on identified psychological and emotional issues.

  • Anxiety, anger management, depression, pain, stress reactions, sleep difficulties, eating disorders, social anxiety, Irritable Bowel Syndrome, obsessive compulsive behaviors.

Biofeedback Therapy:

Identifies and corrects the muscular patterns and stress responses

  • Achilles tendonitis, arthritis, bunion, chondromalacia, diabetes, ilio-tibial band, morton’s neuroma, plantar fasciitis, shin splints

Active Release Techniques (ART):

A patented, state of the art system, Active Release Techniques (ART®) is a soft tissue technique used in a variety of situations, including breaking up scar tissue, which can increase range of motion and encourage better blood flow.
